After you've made an appointment, please download, print, and fill out the forms listed below.
If your appointment is in our office, please bring the completed forms with you to your first session. If your appointment is via phone or webcam, please either mail them to the address you were given in your confirmation email, or scan and email them back by replying to your confirmation email.
